Here are the forms that are used SPECIFICALLY by the Houston chapter.
- The NOMINATION FORM, which requires a second nominator
- The WORKSHEETS (A,B,C and D) which must be signed by the nominee and the nominator. Worksheet A is the most important, as there must be a minimum of 10 cases tried to civil jury verdict as lead counsel in order to be considered for membership.
These forms are slightly different than the ones on the ABOTA.org site, as not all chapters require a second to each nomination.
